Where can I purchase a Peavey Bass Mark IV Series remote switch jack?

My haad came with the jack completely missing. This from a picture of the net

How is this switch wired? I don't see it on the schematic.

I don't see a picture, but I think they used a standard 6 pin DIN, if I'm not mistaken.
