does the ground/shield get soldered to the jacks outer shell at both ends or just one end?

both ends. The isolation occurs on the pan itself (esp. if its an Accutronics pan - you should notice that one of the RCA sockets on the pan is mounted on a recessed hunk of sheetmetal - that is the ground-isolated bit)Building a better world (one tube amp at a time)
