Tubes warm but not hot?

What could be the cause of vacuum tubes in an amp being warm but not hot? Occasionally I come across an amp that I can actually touch the output and preamp tubes when the amp is on. Most of the time the amp tubes are scorching burn-your-skin-off hot!
