Sounds like one of the many CAnadian amps from the 60s/70s. There shouldn't be much to it...it'd be easy to trace it out and make your own schemo.

I think it was made by Sound Projects in Chicago, who also made Lectrolab. It is probably exactly the same as a Lectrolab R600B and Harmony H306A. See both of those schematics in the photo gallery here:
