    I'm having a little hard time following your wiring.... old man eyes doesn't help. Some close ups would help.
    Your voltages on the preamp tubes is a little low but not God awful low.
    The screen voltage is a bit too low and it seems like something is drawing a little too much current across the 22K resistor in the B+ rail, assuming it is a 22K resistor.
    Check to see if your 12AX7 socket is actually wired correctly with respect to the lugs 6, 7, and 8 and their supporting circuitry.
    I can't tell, because of the smallish picture and the angle, but make sure lug 6 is on the 100K resistor with the first 100nF phase inverter coupling cap, lug 7 is connected to the 1M resistor and the cathode, lug 8 is connected to the 56K resistor with the other 100nF phase inverter coupling cap.
    Also, double check to see if the .022uF cap is connected to the 1M resistor next to your filter caps.
