Has anyone experienced an issue with wharfedale titan 12D keeps burning TDA8954TH.

The TDA8954TH.is a pretty robust output IC.
It incorporates all sorts of self monitoring and protect schemes.
Thermal contact issues with the heat sink tab will fry them quick.
