The amp in your photo is an "RV" model amp with reverb and vibrato. At the time this amp was made, Kustom would use the list price of the amp as the model number. So your amp might be a model 595RV or a 795RV, etc.

The top edge of the cabinet is wider and so this is often referred to a wide panel or a Frankenstein head.

If you go to the Vintage Kustom site, they have a schematic for this amp. You will find it incorrectly listed under the K200-A listings, but the sheet will be marked as "Wide Panel".
